The Monk Strong Memorial Foundation is dedicated to honoring Justin’s legacy by supporting members of our community — including first responders, military personnel, teachers, nurses, and others — who have fallen on hard times.

We know that many of you personally know someone who could benefit from this support. We invite you to submit a nomination so that we can continue blessing families and individuals who need a helping hand.

Once completed, you can return the form to us by email or by mail to the address listed on the form. Our board will carefully review each nomination, and we will contact you if additional information is needed.
